New Service, Automatic Payment and Other Applications

Our most commonly used forms and applications are available below for download, viewing or printing. All of these documents are also available at the MVWD main office.

About These Applications

Residential Service Application: Please print the appropriate application and call the office at (702) 397-6893 so that a district employee can go over the application with you.
There is a $25.00 non-refundable application fee.
You will also need to bring in your drivers license and provide proof of ownership (i.e. Deed of Trust, Bill of Sale, Closing Statement). Property owners renting their property must fill out an Owner Authorization Form in order for the renter to put the water into their name.
In order for a meter to be turned back on the same day, the following items must be received no later than 3:00 pm: Application, driver’s license, proof of ownership or Owner Authoriztion Form.

Construction Water: When applying for a hydrant meter, the applicant will need to bring in their dust permit issued by Clark County. Before the hydrant meter can be installed the backflow device on the water truck must be inspected by district personnel.

Employment Application: The District will post open positions in the local newspaper and on this website in the job listing section under public info. The District will take applications even when there are no positions open and will keep them on file for one year. If you have an employment application on file and see a position open that you are interested in, you will need to call the District and have an employee pull your application and put it in the file for the open position.

System Flow Capacities

This form gives you the number of fire hydrants within 500′ of your parcel boundaries and the water distribution system flow capacities for the general area of your parcel. This is described by a static pressure (Pstatic) and a calculated flow at 20 psi (Q@20 psi). These parameters along with the dwelling size provide the Clark County Fire Department with necessary information to make its decision on what you will have to do to meet CCFD’s requirement for fire suppression. Moapa Valley Water District does not have fire suppression requirements as a condition of service.
